Kiama/Shellharbour Community Options is a service specifically for people with a disability, dementia or the frail aged. It includes Case Management and brokerage service for people who are eligible for HACC funded services however have complex and multiple care and support needs. Some of the case management projects available include Dementia, CALD, and ComPacks (external referral process).

Case Management is an approach to service delivery which attempts to ensure that individuals’ with complex and/or multiple problems and disabilities receive the services that they need in a timely fashion. Case Management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet individuals’ needs through communication and available resources to promote quality cost-effective outcomes. Wherever possible, Case Management should facilitate the development of individuals’ skills to take control over their own health needs so that individuals are able to demonstrate control over their lives.
